The population of the areas of El Gorg, Sant Roc, Llefià, La Salut and Sant Adrià de Besòs (CAP Dr. Barraquer, 132,034 inhabitants)
Control of cardiovascular risk factors:
Vascular education program by Cardiology nurses aimed at patients discharged from the Germans Trias i Pujol University Hospital with a recent diagnosis of ischemic heart disease.
Heart Failure:
“De novo” diagnosis of heart failure, systolic or diastolic, to initiate medical treatment, carry out appropriate complementary examinations and follow-up until stabilization.
Control and monitoring of patients with heart failure due to known systolic, valvular or coronary dysfunction, dilated or hypertrophic cardiomyopathy.
Decompensation of previously stable heart failure symptoms, despite usual treatment.
Ischemic Heart Disease:
Initial diagnosis of chest pain compatible with coronary disease in patients with risk factors, to provide appropriate treatment, complementary examinations and monitoring until stabilization.
First controls of patients discharged from hospital and diagnosed with acute myocardial infarction or unstable angina, whether or not they have undergone percutaneous or surgical revascularization.
Stable patients discharged from the hospital specialist consultation.
Control and monitoring of all patients with a previous heart attack or stable exertional angina.
Valvulopathies:
Control and monitoring of valvular diseases with a degree of severity (moderate-severe) requiring surgical treatment.
Control and follow-up of valvulopathies of moderate degree.
Patients with previously stable, untreated valvular heart disease who present a change in their functional class or the appearance of new cardiac symptoms.
Follow-up after valve replacement surgery.
4-week follow-up of patients undergoing valve replacement surgery just after hospital discharge.
Arrhythmias-Syncope:
Initial diagnosis and appropriate complementary examinations to rule out structural heart disease and treatment of paroxysmal or persistent atrial fibrillation (need for pharmacological or electrical cardioversion).
Reassessment of any symptoms of cardiac decompensation in previously stable patients with permanent atrial fibrillation.
Initial diagnosis of palpitations suggestive of tachyarrhythmias.
Study of syncope with cardiogenic characteristics, to carry out appropriate examinations, assessing the need for a pacemaker.
Miscellaneous:
Control and monitoring of stable hypertrophic cardiomyopathy.
Diagnosis and monitoring of pericardial diseases such as: chronic pericardial effusion, acute pericarditis, constrictive pericarditis stabilized with medical treatment.
Control and monitoring of congenital heart diseases, intervened or not, that remain stable, as well as those that vary their functional class or symptoms of decompensation appear in order to assess the need for medical and/or surgical interventions.
